Chandler-Taniguchi stabbed four people, including his grandfather, with a knife after being released from the psychiatric ward at the Mahelona Medical Center. One elderly victim died from her injuries after being stabbed. Police confronted Chandler-Taniguchi while he tried to attack two other bystanders. He then turned on an officer, who shot and killed him.
